Enrollment process for the Complementary Health Care Program

Service Benefit Plan Members can enroll in this program in one of several ways:

  1. Call WholeHealth Networks at: 1-877-258-7283 from 8:00 am to 8:00 pm Eastern Time, Monday through Friday. You may pay for your annual membership over the phone with any major credit card or debit card.
  2. Download an enrollment form from this site and mail or fax it with your payment to WholeHealth Networks. You must have Acrobat Reader to view the enrollment form. If you do not have Acrobat Reader click the icon below to down load a free copy of the software.
